Coolbinia Primary School Application for Enrolment Form

When lodging your Application for Enrolment you will need to provide

  • the child’s birth certificate
  • an up to date Immunisation History Statement which must be obtained from the Australian Immunisation Register (visit the Department of Health for instructions on how to obtain this statement)
  • 2 types of proof of address – these must be current within 3 months and may include a utility bill, rates notice, tenancy agreement, internet connection. (Please note that we do not accept a driver’s license.)
